Job Description

Role & responsibilities

Patient Care & Medication Management

  • Conduct patient assessments and review medical histories.
  • Recommend appropriate medications and treatment plans.
  • Monitor drug therapy for efficacy and safety.
  • Identify and manage adverse drug reactions and interactions.
  • Participate in ward rounds and treatment chart reviews.

Education & Counseling

  • Provide medication education to patients and caregivers.
  • Counsel patients on dosage, side effects, and adherence.

Clinical Documentation & Reporting

  • Document clinical interventions and maintain patient progress notes.
  • Maintain accurate records of medication-related activities.

Collaboration & Protocol Development

  • Work with healthcare teams to develop treatment protocols.
  • Participate in medication reconciliation and therapy management programs.
  • Contribute to hospital formulary development and drug safety policies.

Quality & Compliance

  • Ensure compliance with NABH and NAQS standards.
  • Participate in infection control and antimicrobial stewardship initiatives.
  • Support automated drug dispensing systems and inventory control.

Preferred candidate profile

Doctor of Pharmacy (PharmD) or M.Pharm (Clinical Pharmacy)
