Assessment and Diagnosis Conduct initial psychological assessments and intake interviews. Identify substance use disorders, co-occurring mental health conditions, and behavioral issues. Create and maintain individualized treatment plans based on patient needs. 2. Individual and Group Counseling Provide one-on-one therapy sessions focusing on addiction recovery, trauma, and emotional wellness. Lead group therapy sessions using evidence-based approaches (CBT, DBT, 12-step facilitation, etc.). Guide patients in understanding their triggers, behaviors, and coping strategies. 3. Family Counseling and Support Involve family members in the recovery process through education and counseling. Address family dynamics that may contribute to addiction. Facilitate family therapy sessions where appropriate. 4. Relapse Prevention Planning Help patients identify high-risk situations and develop relapse prevention strategies. Teach life skills, stress management, and healthy coping mechanisms. Prepare patients for reintegration into society with tools for long-term sobriety. 5. Documentation and Case Management Maintain accurate records of counseling sessions, progress notes, and treatment plans. Coordinate with medical staff and other professionals to ensure a holistic approach. Monitor progress and update treatment goals regularly. 6. Education and Psychoeducation Educate patients about addiction, recovery, mental health, and related topics. Conduct workshops or seminars on emotional regulation, self-esteem, anger management, etc. 7. Crisis Intervention Provide immediate support during emotional breakdowns, relapses, or suicidal ideation. Implement de-escalation techniques and coordinate with medical staff for emergency interventions. 8. Ethical Practice and Confidentiality Maintain professional boundaries and ethical standards. Protect patient confidentiality and respect their dignity at all times. Follow legal and regulatory requirements in handling sensitive information. 9. Motivation and Empowerment Encourage and motivate patients to take ownership of their recovery. Foster hope, accountability, and personal growth.
