Position Summary: The Transplant Coordinator is a specialized healthcare professional responsible for managing all aspects of care for patients undergoing organ or tissue transplantation. This includes coordinating evaluations, donor matching, surgical scheduling, and post-transplant follow-up. The coordinator acts as the primary liaison between patients, families, the transplant team, and external agencies to ensure safe, timely, and effective transplant care. Key Responsibilities: Patient Coordination: Guide patients through the transplant process from referral to post-transplant follow-up. Educate patients and families about transplant procedures, medications, and lifestyle modifications. Assess patient readiness and ensure completion of all required evaluations. Clinical Management: Collaborate with multidisciplinary teams (surgeons, physicians, nurses, pharmacists, social workers, etc.) to develop and implement individualized care plans. Monitor patient progress and coordinate pre- and post-operative care. Participate in selection committee meetings and document patient eligibility decisions. Donor Coordination: Facilitate communication between donor programs, surgical teams, and recipients. Verify donor-recipient compatibility, ensure regulatory compliance, and assist in organ procurement logistics. Documentation & Compliance: Maintain accurate medical records in accordance with hospital policy and national regulatory standards (e.g., UNOS, OPTN). Ensure timely reporting and data submission to transplant registries. Education & Quality Improvement: Provide education to staff and the community about transplantation.
