Key Responsibilities Medical Record Analysis: Conduct thorough reviews of complex inpatient medical records, including physician's notes, reports, and charts, to identify all relevant diagnoses and procedures. Code Assignment: Accurately assign and sequence ICD-10-CM (diagnosis) and ICD-10-PCS (procedure) codes based on clinical documentation and official coding guidelines. DRG Assignment: Utilize encoder software (e.g., 3M, Epic) to assign the correct MS-DRG or APR-DRG based on the assigned codes and other patient data (age, severity, etc.).
