Job Description

  • Registrar shall be responsible to attend to any new admission immediately within 30 minutes from the time of admission, explain to the patient about the proposed care in the hospital & document the same within 2 hours.
  • Registrar shall be responsible for taking a proper history of the patient from the patient or from the care taker, sign the same with name in block letters, time and date.
  • Registrar shall be responsible for the physical examination of the patient after taking the medical history of the patient.
  • Registrar shall be responsible to record all the details in the case sheet.
  • Registrar shall be responsible to obtain ''Informed Consent form'' from patient's relatives.
  • Registrar shall be responsible to inform the Consultant about the patients condition as and when necessary and get further orders.
  • Registrar shall be responsible to carry out emergency and necessary treatment to stabilize the patient.
  • Registrar shall carryout regular rounds at appropriate time.
  • Registrar shall be responsible to instruct the nursing staff about the necessary orders to be carried out.
  • Registrar shall be responsible to ensure that the appropriate investigation is done for the patient as per the admitting Consultant's order.
  • The Registrar can order for investigations if the situation warrants.
  • Registrar shall be responsible to write the drug prescription legibly with the details of full name of patient, age, sex, Hospital ID no, name of the DMO, time, date with signature.
  • Registrar shall be responsible to check the dosage, frequency, route of administration of the prescribed drugs and sign the same with name in capital letters, time and date.
  • Registrar shall be responsible to go on rounds, examine the patient and inform the respective Consultant if there is any new finding.
  • Registrar shall be responsible to accompany the Consultants on rounds and carry out the necessary changes in their order.
  • In case of patient status deterioration in wards, Registrar will take initiative in assessment, initial care, co-ordinate with ICU for transfer.
  • Registrar shall respond in timely manner to nurses calls for patient assessment if unstable patients identified on wards.
  • Registrar shall track pending labs during their shift, enter in charts and inform abnormal levels to consultants.
  • Registrar shall be responsible to fill in the pre-authorization form if the patient is a TPA patient.
  • Registrar shall be responsible to prepare the immediate discharge document at the time of patient's discharge and prepare the original discharge summary within 24 hours.
  • To accompany the patient when the patient is taken for CT Scan.
  • Registrar shall be part of the training program conducted in the hospital and to take continuous initiatives to upgrade their skills and knowledge.
  • Registrar shall be a part of the Quality Programs and take initiatives in the development, implementation and amendments of various clinical policies.
  • Registrar shall be responsible to take active part when there is a '' CODE BLUE '' situation.
  • Registrar shall take part in the medical camps as and when required.
  • Registrar shall take part in the research activities related to

his/ her job.
