Job Responsibilities: Conduct Assessments: Evaluate emotional, social, and practical needs of residents. Community Linkages: Connect residents to healthcare, education, and employment resources. Rehabilitation & Reintegration: Design and implement personalized rehabilitation plans. Field Work: Perform home visits and community outreach for follow-up support. Skill Training: Organize vocational and life skills development sessions. Awareness Programs: Educate residents on health, rights, and life skills. Documentation: Maintain case files, progress reports, and rehabilitation plans. Advocacy: Represent residents needs and rights with external agencies.
