We are seeking a compassionate, dedicated, and experienced Registered Nurse (RN) to join our Home Health team. In this role, you will provide skilled nursing care to patients in their homes, ensuring high-quality and individualized care. You will work collaboratively with a multidisciplinary team to support patient recovery, independence, and quality of life. Areas of Coverage: Metro Detroit, Down River, Redford, Westland, Livonia, Southfield, Novi, Canton, Warren, Sterling Heights, Pontiac, Waterford Key Responsibilities: Conduct in-home assessments and develop individualized care plans Administer medications, wound care, IV therapy, and other nursing procedures Monitor patient health and progress, and update care plans accordingly Educate patients and families on managing illness or injury and using medical equipment Coordinate with physicians, therapists, and other healthcare professionals Maintain accurate clinical documentation in accordance with agency policies and regulations Ensure compliance with state, federal, and home health regulations (e.g., Medicare/Medicaid) Participate in case conferences and performance improvement initiatives Qualifications: Licensed Registered Nurse (RN) in the state of Michigan 1-2 years of clinical nursing experience, preferably in home health, acute care, or hospice Knowledge of OASIS documentation is highly preferred Strong clinical, organizational, and communication skills Valid driver’s license and reliable transportation CPR certification (current and valid) Preferred Skills: Experience with electronic medical records (EMR) systems Ability to work independently with minimal supervision Adaptable and comfortable working in diverse home settings Powered by JazzHR MXuyg52rHw
