Job Overview We are seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join our healthcare team. In this role, you will provide specialized care to patients with serious illnesses, focusing on improving their quality of life through symptom management, emotional support, and coordination of care. You will work collaboratively with physicians, nurses, and other healthcare professionals to deliver comprehensive palliative care services. Duties Conduct thorough assessments of patients to determine their physical, emotional, and spiritual needs. Develop and implement individualized care plans in collaboration with the healthcare team. Manage symptoms associated with serious illnesses, including pain management and psychosocial support. Maintain accurate medical documentation and medical records in compliance with HIPAA regulations. Educate patients and their families about treatment options, disease progression, and palliative care principles. Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes. Advocate for patients' needs and preferences within the healthcare system. Stay current with medical terminology, anatomy knowledge, physiology knowledge, and advancements in palliative care practices. Experience Candidates should possess the following qualifications: - Nursing degree (BSc or MSc) with palliative care experience - Certification in palliative care.
