Job Description

Roles & responsibilities: -

Home visits as part of transactional care and long-term care
Screening for Transactional Nurse visit
To deliver outstanding clinical care guided by evidence-based medicine and ethical practices
To monitor the care delivered and provide the necessary clinical leadership to facilitate better recovery
To provide Goal based care for patients resulting in better outcomes
To implement clinical care protocols and pathways for common home healthcare presentations
Conduct clinical assessment Physically and Virtually of Patients
Perform basic/advance clinical procedures as requested by patients
Documentation compliance of mandatory clinical records
Conduct training for nurses and support staff as per the requirement
Adherence to complete the consultations as per TAT
To conduct medical fitness assessment for AHC employees
Any other task assigned by Lead Physician
