Job Description

Participate in ward rounds with physicians to provide pharmacological input on drug therapy.
Review and monitor patient medication charts for appropriateness, interactions, and dosing accuracy.
Support the development of individualized pharmacotherapy plans, especially for complex cases.
Conduct
Therapeutic Drug Monitoring (TDM)
and interpret pharmacokinetic data.
Provide clinical advice on
adverse drug reactions (ADR)
and
drugdrug interactions
.
Participate in
antimicrobial stewardship
and
formulary management
activities.
