Job description Role & responsibilities Participate in ward drug rounds on a regular basis, scan case sheets and monitor adverse drug reaction, drug-drug & drug-food interaction. Improve the prescriptions and drug chart making work simpler for doctors and nurses. Perform pharmacy audits to the prevention of medication errors. Perform chemotherapy drug mixing. Promote improvement in drug usage/dosage and in the supervision of therapeutic procedures for best results. Educate staff about drugs and diseases through structured training and providing information of drugs. Preparing the Chemo admixtures whenever required according to the protocols. Ensure safety in compounding, preparing, and dispensing chemotherapy. Responsible for minimizing drug waste, managing drug shortages, and decreasing exposure to hazardous drugs. Counsel patients on the use of medications. Review and maintain drugs policy. Organize pharmacy and therapeutics committee meeting at regular intervals to review and monitor drugs use Preferred candidate profile Candidates with min 2 year's experience in hospital is preferred. Pharm D graduates with TN Pharmacy Council Registration can apply.
