Clinical Pharmacist

As a Pharmacist in this role, you will be responsible for assessing the status of patients" health problems and evaluating the appropriateness and effectiveness of their medications. Your primary focus will be to ensure that prescribed medications meet the patients" needs and goals of care optimally. It will be crucial for you to follow the patients" progress closely to determine the impact of medications on their health and advise them on the best practices for medication administration. In addition to medication management, you will play a key role in supporting healthcare teams in educating patients on steps to improve or maintain their health, including advice on exercise, diet, and preventive measures like immunization. Your specialized knowledge of medication science and clinical use, including medication action, dosing, adverse effects, and drug interactions, will be essential in collaborating with other healthcare professionals to deliver comprehensive patient care. You will be required to have a detailed understanding of various drugs and their effects, exercise control over prescribing medications, and monitor patients closely to ensure optimal outcomes. Your responsibilities will include selecting appropriate drugs, diagnosing untreated illnesses, consulting with patients on medication effects, and ensuring patient adherence to prescribed drug regimens. Collaboration with physicians, healthcare professionals, and patients will be a key aspect of your role to ensure that prescribed medications contribute to the best possible health outcomes. Moreover, you will be an integral part of the healthcare team and will assist in conducting audits, both medical and surgical, under the guidance of the Healthcare Improvement Committee (HIC) chairperson. The ideal candidate for this position should possess a Pharm.D. qualification and have 1 to 3 years of experience, preferably in a hospital setting. This is a full-time position with a day shift schedule, and prior experience in a hospital environment would be advantageous.
