About the Project Abhivyakti Foundation is implementing a Targeted Intervention (TI) Project for Injecting Drug Users (IDUs) in Amritsar district with the support of Punjab State AIDS Control Society (PSACS). The project provides comprehensive HIV prevention and care services including harm reduction, STI/RTI management, linkages to Integrated Counselling and Testing Centres (ICTC), Anti-Retroviral Therapy (ART) centres, and psychosocial support. The project also focuses on community mobilization, counselling, condom promotion, safe injecting practices, and capacity building of outreach teams to reduce the spread of HIV/AIDS among HRGs. Roles & Responsibilities of Visiting Physician The selected candidate will be responsible for providing clinical and counselling services under the TI Project as per NACO/PSACS guidelines. Key functions include: Conducting regular health check-ups of IDU beneficiaries ensuring privacy, confidentiality, and dignity of patients. Diagnosing and treating STI/RTI cases using Syndromic Case Management (SCM) protocols. Maintaining accurate medical and clinical records in compliance with project requirements. Counselling patients on prevention, treatment adherence, HIV testing, ART linkages, and risk reduction practices. Ensuring proper usage and availability of clinic equipment (examination bed, instruments, adequate lighting, etc.). Motivating patients to utilize government health facilities for diagnostic and treatment services. Establishing referral linkages with ICTC, PPTCT, ART Centres, DMC, and PLHIV networks. Conducting health education sessions to promote condom use, safer injecting practices, and treatment compliance . Abhivyakti Foundation Care and Support Centre CSC 2.0 Project Name – Shakti Location: Flat No. 140-C, Second Floor, H Pocket, Dilshad Garden, Near Pammi Sweets, Shahdara, Delhi 110095 Position Title: Health Worker (CLH) Educational Qualification and Eligibility: The candidate should possess a minimum educational qualification of 10th pass (Matriculation). Preference will be given to those who have completed 12th standard or hold a graduate degree. Individuals who are living with HIV (PLHIV) or those who have prior experience of working with the HIV community will be given preference for this position. The applicant must have good communication and interpersonal skills and should be comfortable working directly with community members, patients, and health service providers. Candidates should be willing to work extensively in the field, visit beneficiaries regularly, and maintain consistent contact with the target population. It is essential that the candidate has a basic understanding of ART (Anti-Retroviral Therapy) treatment, its process, adherence importance, and the ability to understand and complete reporting formats accurately. Key Roles and Responsibilities: Field Visits and Home Visits The Health Worker will be required to conduct regular field visits and home visits to meet HIV-affected beneficiaries. They will assess the health condition, treatment adherence, and level of family support for each patient.Beneficiaries who have discontinued treatment or missed follow-ups must be traced, motivated, and reconnected to the ART treatment program. Tracking LFU and MIS Clients The Health Worker will identify and track patients who have defaulted from ART centers or are inactive in the MIS (Management Information System). The worker will contact such beneficiaries either through home visits or telephonic communication, understand the reasons for treatment interruption, and facilitate their re-linkage to ART services. Coordination with ART Centres The Health Worker must maintain close coordination with ART centres, ensuring proper communication between beneficiaries, counsellors, and medical staff. They will visit ART centres to update reports, collect or verify data on treatment adherence, and attend review or coordination meetings whenever required.The Health Worker will provide counselling and emotional support to people living with HIV and their families. They will educate them about the importance of regular treatment, adherence to ART medication, and maintaining a healthy lifestyle.In cases of stigma, discrimination, or mental stress, the worker will offer necessary psychological support and refer the case to appropriate support systems when needed.The Health Worker will be responsible for maintaining detailed and accurate records of all beneficiaries under the project. Data must be regularly updated, verified, and reported in the prescribed formats.The worker must ensure timely submission of MIS reports to the CSC project team every month and maintain data confidentiality and integrity.The Health Worker will actively participate in health camps, awareness campaigns, and community meetings organized under the Shakti project. They will support the CSC team in planning, mobilizing community members, and ensuring effective participation of beneficiaries in these programs.The Health Worker must conduct regular follow-up visits to track the progress of each beneficiary. In case a patient has relocated or lost contact, efforts should be made to trace them and re-establish treatment linkage.Daily field monitoring and interaction with the community will be an integral part of the job to ensure treatment continuity and emotional well-being of beneficiaries. Salary and Allowances: Counselling and Support Services Data Collection and Reporting Participation in Health Camps and Community Events Regular Follow-up and Field Monitoring Monthly Salary: Rs. 9000 Travel Allowance: Rs. 5000 Mobile Reimbursement: Rs. 250 Total Approximate Honorarium: Rs. 14250 per month (based on performance and attendance) The total remuneration is subject to satisfactory field performance, regular attendance, and timely submission of reports. Nature of Work: This is a full-time, field-based position that involves extensive travel within the assigned project area. The Health Worker will spend most of the working hours in the community, conducting visits, counselling beneficiaries, and submitting daily activity and progress reports to the CSC team.
