Role & responsibilities Job Responsibilities To review and maintain Medical records as per classification of NABH and Hospital guidelines. Responsible to organize Medical records, validate accurate data entry, retrieving data, scanning documents and creating data backups for the Hospital Maintain colour coding of Medical records, files, assembling, coding, proper maintenance, storage and retrieval of records To classify with ICPM of Health records To collaborate with the Medical, Nursing and other Healthcare staff in completing patient medical records. Ensure the results of treatments and patient's progress are documented accurately. Maintain records for decision making and research purpose regarding patient's care and treatment To submit Birth and death information online after proper validation of Birth and Death reports. Maintain registers accurately. To send Communicable Disease report by mail to corporation officials and share data when requested by Govt officials after the approval of Medical Superintendent/Department HOD. To be accountable and assist in quality assurance, utilization review, infection control, and other committees and programs. Responsible to maintain medical reports, disease reporting, medical certificates and to notify concerned authorities in duly completing the required procedures. To prepare monthly statistical reports and submit to concerned authorities To upkeep medico legal records under safe custody and to attend court whenever required. To share patient files in accordance with the established procedures for medical care, medical education, medical training, medical care evaluation management, and legal purposes .Effectively control the movement of patient files in order to achieve a unit record. Liaise with statutory board/personnel to update approved data/reports To implement Quality Improvement activities and maintain confidentiality of records and data. Preferred candidate profile Must have BACHELOR OF MEDICAL RECORD SCIENCE (BMRSc) degree with relevant work experience in a Multispeciality Hospital Must have minimum 15+ yrs of Experience in MRD and at least 5+ yrs in Senior position Knowledge about Coding and Classification of records Skills related to Medical record assembling, indexing, retrieval, archival and Data management Accurate Documentation skills Good Communication & Liaison skills Job Type: Full-time Work Location: In person Expected Start Date: 10/06/2025
