As a Diabetes Educator, your primary responsibility will be to provide self-management training and education to type 1, type 2, and gestational diabetic patients. This includes educating them on signs/symptoms, as well as the respective treatments for hyperglycemia and hypoglycemia. You will also be involved in offering nutritional counseling for diabetes management. Your role will also involve converting free trial patients into paid patients and being open to taking on new roles within the organization. You will deliver patient education as needed or as directed by the Provider, perform assessments, and collect data to establish outcome criteria in collaboration with patients, family members, and the healthcare team. Additionally, you will attend outreach activities such as health fairs for diabetes screening and health education. Monitoring and evaluating short and long-term patient responses to therapeutic interventions, providing necessary follow-up for patients and families, and educating and following up with patients on controlling blood sugar, lipids, renal function, and preventing diabetes complications will also be part of your responsibilities.
