Key Responsibilities Conduct psychosocial assessments of patients and families. Develop individualized care plans in collaboration with the multidisciplinary team. Provide counseling, emotional support, and guidance to patients and caregivers. Facilitate group sessions, family meetings, and psychoeducation programs. Assist in rehabilitation planning and reintegration of patients into society. Coordinate referrals to external agencies and community resources.
