Role & responsibilities

• Daily audit of prescription and recommendation.
• To check error-prone abbreviations in the prescription.
• Adverse drug reaction/ Medication Error audit.
• Reporting to PVPI.
• Blood Transfusion Reaction/ Metal Implant Reaction audit and report to be sent to the Quality department.
• Promoting safer and more effective use of drugs.
• Rational introduction and use of new and expensive medications.
• Training of healthcare staff in promoting the rational use of medicine.
• Data Validation of Pharmacy Indicators
• Medicine and Consumable audit
• Narcotic Medicines Storage, usage and return of narcotics.
• Help in Pharmacy Manual and formulary documentation.
• To check medicine reconciliation
• Active participation in Pharmacotherapeutic and HIC committee.
• Organizing Pharmacotherapeutic committee meeting.
• Coordination in antibiotic stewardship programme.
• Antibiotic utilization audit
• Outcome of antibiotic reporting
