Job roles & responsibilities :

Review and evaluate medication orders for accuracy, appropriateness, and potential interactions.
Monitor patient responses to medications, including side effects and therapeutic effectiveness.
Adjust medication regimens as necessary based on patient-specific factors and clinical guidelines.
Provide patient education on medication use, including proper administration, potential side effects, and interactions.
Address patient questions and concerns regarding their medications.
Monitor and report adverse drug reactions and medication errors.
Provide education and training to pharmacy staff, healthcare professionals, and patients on medication management and therapy.
Stay current with advancements in pharmacotherapy and participate in ongoing professional development.
