Job Summary:

The Consultant Home Care Nurse is responsible for providing professional nursing services to patients in their homes. The role includes assessment, planning, implementation, and evaluation of patient care. The nurse will work collaboratively with physicians, family members, and other healthcare professionals to ensure high-quality, patient-centered care.

Key Responsibilities:

• Conduct in-home assessments of patients and document clinical status.
• Develop and implement individual care plans based on patient needs.
• Administer medications, wound care, injections, IV therapy, catheter care, etc.
• Educate patients and caregivers on disease management, medication adherence, and hygiene.
• Monitor and document patient progress and update care plans as needed.
• Coordinate with physicians and healthcare teams to ensure continuity of care.
• Maintain accurate clinical records and reports in compliance with company and legal standards.
• Provide emotional support and counseling to patients and families.
• Follow infection control and patient safety protocols.
• Respond to medical emergencies during home visits if required.

Qualifications:

• Education: B.Sc. Nursing / GNM from a recognized institute.
• License: Registered Nurse with State Nursing Council.
• Experience: Minimum 1-2 years of clinical experience, preferably in home healthcare, ICU, or general medical-surgical care.
• Skills:
• Strong clinical and assessment skills
• Good communication and interpersonal skills
• Ability to work independently
• Basic documentation and tech skills (using tablets or apps for reporting)

Working Conditions:

• Flexible schedule based on visits or appointments
• Travel within assigned areas for home visits
• May involve weekend or evening shifts

Remuneration:

• Per visit / hourly / package-based (as per company policy or agreement)

Job Types: Full-time, Permanent

Pay: ₹10,191.15 - ₹44,015.94 per month
