We are currently seeking a Service Coordination Officer to join our busy service coordination team who are the first point of contact for our clients. You will be responsible for organising aspects of the Home Care Package services for our clients, based on assessments completed by the Care Advisors. This role involves liaising with clients, families, providers and other healthcare professionals to ensure that services are delivered efficiently and effectively. Your primary focus will be on providing excellent customer service and maintaining accurate records. To be successful, you will require; Positive attitude: A positive and proactive mindset that is committed to achieving goals, overcoming challenges, and providing excellent customer service. Communication skills: Excellent verbal and written communication skills, with the ability to convey complex information clearly and concisely.
