Key Responsibilities: Pre-Operative Duties: Prepare the OT before surgery by ensuring all instruments, surgical tools, linen, and equipment are sterilized and arranged. Check and maintain OT cleanliness and sterility. Assist in shifting patients to the OT and ensuring proper patient identification and documentation. Verify surgical schedule and patient consent forms. Intra-Operative Assistance: Assist surgeons and nursing staff during surgery by handling instruments and supplies. Operate and monitor equipment such as anesthesia machines, cautery, suction, etc. Ensure all surgical instruments are accounted for before and after the procedure. Maintain aseptic techniques and infection control protocols during surgery. Post-Operative Duties: Assist in patient shifting from OT to recovery. Clean and sterilize instruments and OT equipment after each procedure. Dispose of biomedical waste as per hospital norms. Prepare OT for the next surgery. Equipment Management: Perform routine checks and maintenance of OT equipment. Report malfunctioning or defective instruments/equipment immediately. Record Keeping and Documentation: Maintain inventory of OT instruments, materials, and consumables. Assist in maintaining OT registers, checklists, and sterilization logs. The MRD Executive is responsible for maintaining, updating, and securing all patient medical records as per hospital policies and statutory guidelines. The role ensures accurate data entry, safe record storage, timely retrieval of files, and compliance with confidentiality standards. **2. Key Responsibilities A. Medical Records Management Maintain and update inpatient and outpatient medical records accurately. Ensure proper filing, indexing, and tracking of all medical files. Retrieve patient records promptly on request from clinicians, nursing staff, or management. Ensure completeness of patient files including consent forms, discharge summaries, reports, etc. B. Data Entry & Documentation Enter demographic and clinical data in HIS software. Ensure correct ICD coding for diagnoses and procedures (if trained). Prepare daily/monthly reports related to admissions, discharges, and deaths. C. Compliance & Confidentiality Ensure compliance with NABH / statutory guidelines for medical records. Maintain confidentiality of patient information at all times. Safeguard records from damage, unauthorized access, and misuse. D. Record Storage & Preservation Organize physical and digital records systematically for easy traceability. Monitor old records for weeding/destruction as per retention policy. Ensure proper labeling, barcoding, and tracking of files. E. Coordination Coordinate with wards, billing, laboratory, radiology, and other departments for record completion. Follow up with doctors for pending discharge summaries and deficiencies. F. Additional Responsibilities Support medical audits by providing requested records. Assist in medico-legal case file management. Participate in departmental meetings and quality improvement initiatives. **3. Skills & Competencies Good knowledge of medical terminology. Basic understanding of ICD coding (preferred). Proficiency in hospital software (HIS). Strong organizational and filing skills. Attention to detail and accuracy. Ability to maintain confidentiality.
