Job Description

Pranaam Hospitals is looking for a detail-oriented and efficient Discharge Summary Typist to prepare accurate patient discharge summaries and medical documentation in coordination with doctors and other hospital departments.

Key Responsibilities

• Type and prepare discharge summaries based on doctors’ notes, dictation and clinical records.
• Ensure accurate entry of patient details, diagnosis, treatment, procedures, medications and follow-up instructions.
• Proofread summaries for spelling, grammar, formatting and completeness.
• Coordinate with consultants and treating doctors to clarify missing or unclear information.
• Ensure timely completion of discharge summaries to avoid delays in the patient discharge process.
• Update and maintain discharge documentation in the Hospital Information System (HIS/HMS).
• Coordinate with nursing, billing and Medical Records Department (MRD) teams.
• Maintain patient confidentiality and secure medical records.
• Follow hospital documentation policies and applicable NABH requirements.
• Ensure all summaries are submitted for review and approval by the authorized doctor.

Candidate Requirements

• Qualification: Any Degree or Diploma; medical transcription training is an advantage.
• Experience: 1–3 years of experience in discharge summary typing or medical transcription preferred.
• Good English typing speed and accuracy.
• Knowledge of medical terminology and hospital documentation.
• Proficiency in MS Word and basic computer applications.
• Ability to work under pressure and meet deadlines.
• Good communication, coordination and attention-to-detail skills.
• Willingness to work in hospital shifts as required.
