Patient Referrals Issue clear and descriptive referral slips for patients requiring higher-level care. Maintain detailed records of referrals, including reasons, patient details, and emergency notes, in both the referral slip and referral register/vehicle logbook. 4. Patient Counselling & Health Education Provide personalized counselling to patients and families on treatment adherence, hygiene, nutrition, and disease prevention. Track and monitor patients with chronic conditions and follow-up needs to ensure continuity of care. Conduct health awareness sessions in the community with the support of the Nurse & Health educator , promoting improved health literacy and health-seeking behaviour. 5. Inventory & Resource Management Supervise and manage the stock of medicines, healthcare products, and reagents to ensure availability without wastage or pilferage. Ensure patients receive optimum benefits from MMU services. 6. Community & Stakeholder Collaboration Coordinate and cooperate with health department staff, local authorities, and key stakeholders to strengthen community engagement and support. 7. Team Supervision & Development Supervise and guide the daily activities of MMU staff, including the Pharmacist, Nurse, and Health Educators. Provide regular, need-based technical training to the MMU team to strengthen skills and service quality. Support and collaborate with the team in delivering effective healthcare services.
