Support Worker

• Emotional and Social Support:
• Building rapport and trust with individuals.
• Offering emotional support and encouragement.
• Promoting social inclusion and participation in community life.
• Helping individuals develop life skills and independence.
• Collaboration and Communication:
• Working with other healthcare professionals, such as nurses and therapists, to ensure coordinated care.
• Communicating effectively with individuals, their families, and other relevant parties.
• Documenting client progress and reporting any concerns.
• Specific Tasks:
• Supporting individuals with learning disabilities to achieve personal goals.
• Assisting young people with transitioning into adulthood.
• Providing overnight support.
• Supporting individuals to find and maintain employment.
