Job Description

Conduct assessments of clients’ needs, situations, and strengths. Develop and implement intervention plans based on client goals. Provide counseling and emotional support to individuals and families. Connect clients with community resources such as healthcare, housing, employment, education, and legal aid. Maintain accurate and confidential case records and documentation. Collaborate with other professionals such as doctors, teachers, and government officials. Monitor and evaluate client progress and adjust plans as needed. Advocate for client rights and access to services. Participate in community outreach, awareness programs, and training sessions. Respond to crisis situations, such as abuse, neglect, or mental health emergencies.
