Job Title: Medical Document Reviewer

Job Summary

The Medical Document Reviewer is responsible for reviewing, analyzing, and validating medical documents to ensure accuracy, completeness, compliance with regulatory standards, and adherence to organizational guidelines. This role supports clinical, legal, insurance, research, or regulatory processes by ensuring high-quality medical documentation.

Key Responsibilities

• Review medical records, clinical documents, case reports, and related healthcare documentation for accuracy and completeness
• Ensure documents comply with regulatory requirements (e.g., HIPAA, FDA, ICH-GCP, CMS guidelines)
• Identify discrepancies, inconsistencies, or missing information and coordinate corrections with relevant stakeholders
• Validate medical terminology, diagnoses, procedures, and coding (ICD-10, CPT, HCPCS as applicable)
• Maintain confidentiality and data security of patient information
• Prepare review reports and document findings clearly and accurately
• Support audits, legal reviews, insurance claims, or clinical research documentation as required

Required Qualifications

• Bachelor’s degree in Life Sciences, Nursing, Pharmacy, Health Information Management, or related field
• Strong knowledge of medical terminology and clinical documentation
• Experience reviewing medical records, clinical data, or healthcare documentation
• Familiarity with healthcare regulations and compliance standards
• High attention to detail and strong analytical skills
• Proficiency in MS Office and document management systems
