Position Overview A Hospital Pharmacist is responsible for ensuring the safe, effective, and rational use of medications within a healthcare facility. They work closely with physicians, nurses, and other healthcare professionals to optimize drug therapy, enhance patient outcomes, and maintain compliance with hospital policies and regulatory standards. Key Responsibilities1. Medication Management Review and interpret medication orders for accuracy, appropriateness, and potential drug interactions. Dispense medications in accordance with legal and hospital guidelines. Prepare sterile and non-sterile pharmaceutical products, including IV admixtures, chemotherapy, and total parenteral nutrition (TPN). 2. Clinical Pharmacy Services Participate in patient rounds and provide evidence-based medication recommendations. Monitor patient therapeutic responses and adjust medication therapy as needed. Conduct medication reconciliation during admission, transfer, and discharge. Provide drug information to healthcare staff and patients. 3. Patient Safety & Quality Assurance Ensure safe medication practices and prevent medication errors. Manage adverse drug reaction (ADR) reporting and medication incident documentation. Assist in antimicrobial stewardship, formulary management, and therapeutic drug monitoring (TDM). 4. Inventory & Supply Management Maintain adequate stock of medications, including controlled substances. Oversee storage conditions and ensure proper handling of medications. Coordinate procurement with suppliers and manage expired or recalled products. 5. Regulatory Compliance Ensure compliance with national pharmacy laws, hospital policies, and accreditation requirements. Maintain accurate documentation and records for audits and inspections. 6. Education & Training Educate patients on proper medication use, side effects, and adherence. Train pharmacy interns, residents, and other healthcare staff. Participate in continuing professional development and research activities. Qualifications Bachelor’s or Doctor of Pharmacy degree (B.Pharm, PharmD, or equivalent). Valid pharmacist license/registration. Hospital pharmacy experience preferred. Strong knowledge of pharmacology, therapeutics, and drug interactions. Skills and Competencies Attention to detail and high level of accuracy. Strong communication and teamwork abilities. Critical thinking and problem-solving skills. Knowledge of hospital information systems and electronic prescribing. Ability to work in a fast-paced and high-pressure clinical environment.
