Summary. GDA Home Care / Transition Care Provides basic patient care and support services in healthcare. Roles And Responsibilities. Helping patients exercise and other physio requirements suggested by therapists. Dressing and feeding patients, the elderly, or the disabled who are under their care. Maintenance of patients safety and health needs. Medical record activities, such as file management and record-keeping. Responsible to maintain the patient data that is to be recorded such as vital signs and measurements of water intake and urine. Responsible to assist patients in getting in and out of bed and walking about a little, as well as move around their rooms. Responsible to give any additional care that is required, such as housekeeping and assisting in the maintenance of a safe environment for patients. Equipment maintenance and safety checks are also the responsibility. Responsible for schedule diagnostic tests, meet and welcoming patients, or provide specialized supplies and drugs. Responsible for ascertaining that the patients diets are nutritious and well-balanced. Must possess the knowledge of equipment like wheelchairs, stretchers, catheters, etc. well versed with the cleaning procedures to control any type of infection. Responsible to possess the knowledge of different types of baths like complete bed-bath, partial bath, tub bath, etc.Adept with the procedures of reporting something that is clinically important.
