Job Description

To attend new patient (admission) immediately

To take proper history of the patient (if he is in a position to give it on his own) or from his relatives

Check any previous record of illness, medication, Drug allergy, if available

Write down all the details in the Initial assessment sheet according to Perform and undersigned after completion of case record

Let the patient or his relative go through all the details and take his signature on the Initial assessment sheet

Inform the respective consultant about the admission, write down on a sheet reconfirm and duly follow his orders

Instruct the nursing staff about the necessary orders to be carried out and cross check

If theres a transfer of inpatient from ICU, read the transfer summary carefully, see the patient, examine him thoroughly and instruct the nurse on duty to carry out the orders

Inform the respective consultants and carry out their orders, inform the nursing staff if any changes need to be made

To take rounds and examine individual patients while joining duty and before completion of duty, inform the respective consultants if any new findings investigation reports and carryout respective orders

To enter the details of each and every patient in the case sheets with time, date of examination and duly signed with full name after completion

Attend rounds with the respective consultants and carry out the necessary changes in the orderEnter them in the treatment sheet; inform the nursing staff on duty about the same

To follow up with consultant all the investigations to be seen and trace their results, inform the Consultant, note down investigation results in the investigation sheet and with special reference to critical values and results

To enter the treatment sheets for the following day, cross check the same for the dosages of the medications and put signature with nameConfirm the investigations to be sent on the following dayTo take special precautions for any special investigations and coordinate with staff nurse on duty

To enter pre-operative orders in the treatment sheet and take proper consents, complete the surgical sheets, check whether right patient is being sent to the OT with proper preparation and medication given and instruct the staff nurse on duty regarding the same

To attend patients complaints, to calm the patients if they are apprehensive, inform the consultant if needed and revisit patients time and again for follow up

To prepare the discharge summary as suggested by the consultant.
