Role Overview: At Arooj, the aim is to be a one-stop-shop for everything that may be required for healthcare support at home. This includes visits by different healthcare professionals such as doctors, nurses, physiotherapists, or healthcare assistants (HCAs), equipment such as mechanized beds, air or water mattresses, oxygen, suction machines, ventilators, etc., or laboratory investigations done from home. Key Responsibilities: - Assess the needs of the patients by taking complete case history from relatives and reading all available health records. - Prepare a nursing care plan based on the patient's needs in collaboration with the Head of Nursing at Arooj and share this plan with the family. - Maintain clinical documentation on a shift basis including Vitals Monitoring Chart, Intake Output, Medication Chart, and provide detailed shift handover to the incoming nurse. - Provide direct patient care including bed making, changing bed sheets, mouth care, back care, bed bath, hair wash, changing position, and fulfilling all basic needs of the patients. - Provide comfort and maintain the safety of the patient while monitoring their condition closely. - Assist patients with activities such as reading, writing, or going for a stroll as needed. - Implement palliative treatments for terminally ill patients as instructed by the family doctor or consultant, which may include pain management with medication. - Maintain a safe home environment by arranging the patient's room and care consumables, ensuring safe use of equipment, cleaning equipment daily, and keeping patient surroundings neat and clean. - Handle patient care equipment and machines, collect biomedical waste separately, discard infectious waste carefully, and ensure the safe disposal of sharps. - Focus on patient safety to prevent falls, pressure ulcers, aspiration pneumonia, catheter-related infections, and provide wound care. - Maintain daily communication with patients, relatives, and treating physicians about the patient's condition.
